Osteoporosis vs. Osteopenia: What's the Difference?

Osteopenia means bone density is lower than normal, but not low enough to be classified as osteoporosis. Think of it as an early warning sign — a window where the right exercise program can meaningfully change your trajectory.

Osteoporosis is a more advanced loss of bone density and structure that increases the risk of fracture, even from everyday movements like bending, lifting, or a minor fall. Both conditions are diagnosed through bone density testing (DEXA scan) ordered by your physician, and both respond well to the right kind of physical activity.

Neither diagnosis means you should avoid exercise. In most cases, the opposite is true: bone responds to load. The right amount of the right kind of stress on your skeleton, applied safely, is one of the most effective tools you have.

How Physical Therapy Helps with Bone Health

Physical therapy for osteoporosis and osteopenia isn't a generic strength class. It's a progressive, individualized plan built around your bone density results, your current fitness level, and your goals. At Valor, that plan typically includes:

  • Progressive weight-bearing and resistance training — bones respond to mechanical load, so we build strength safely and progressively rather than avoiding it
  • Balance and fall-prevention training — since most fractures happen from falls, not from bone fragility alone
  • Posture and spinal-loading strategies — safe ways to bend, lift, and move through your day that protect the spine
  • Movement education — practical guidance on which movements to modify (like certain twisting or high-impact spinal flexion patterns) and which ones to keep doing
  • Objective progress tracking — using tools like VALD performance testing to measure strength and balance gains over time, not just guess at them

We coordinate with your physician or endocrinologist as needed, especially around your DEXA results and any activity restrictions they've recommended.

Yes, in most cases. Exercise, especially weight-bearing and resistance exercise, is one of the most effective tools for maintaining and building bone density. A physical therapist can help you exercise safely by identifying which movements to modify and which to progress.

This depends on your bone density results and fracture risk, which is why an individualized evaluation matters. In general, we help patients modify high-impact spinal flexion and certain twisting movements while still building strength everywhere else.

Physical therapy can't replace medical treatment, but progressive, correctly-dosed weight-bearing and resistance exercise is well supported as part of a bone-health plan. Just as important, PT reduces fracture risk by improving balance and strength, which lowers your chance of falling in the first place.

Valor Physical Therapy is a cash-based clinic, which allows for longer, one-on-one sessions with your provider rather than time split between patients. We can provide a superbill for you to submit to your insurance for possible out-of-network reimbursement.
