physical therapy

If you have ever felt like just another number in a crowded waiting room, you are not alone. Many active adults and athletes in Carlsbad searching for relief from pain or a path back to their favorite activities find themselves stuck in a frustrating cycle. You visit a clinic, receive a generic exercise sheet, and spend most of your session on a heating pad while your therapist bounces between multiple other patients. This assembly-line approach to healthcare lacks the individualized attention required for a true, lasting recovery.

Not only do the patients suffer from this model, but the physical therapists do too. Both Dr. Valentine Ngo, PT, DPT and Dr. Brian Fonseca, PT, DPT have worked in clinics like this in the past. These facilities prioritize quantity over quality, profit over progress, and patient volume over patient care. Physical therapists that work in these settings eventually experience high rates of burnout, especially after treating multiple patients at the same time, seeing 15+ patients per day, and required to do documentation on their own time.

When choosing a physical therapy clinic to go to, think about the kind of care you want for your injury/condition. Here are some aspects worth thinking about before you book your first session.

Individualized, One-on-One Care vs. the Assembly-Line Model

One of the major aspects that patients care the most about is "who am I going to see when I go to physical therapy?"

There are generally two options: one-on-one treatment where you work with the same physical therapist for the entirety of the session OR treatment sessions that consist of working with a physical therapist and a physical therapy aide (typically someone who is working towards getting into PT school or medical school). Obviously, there are pros and cons to both systems.

When you go to a physical therapy clinic that focuses on one-on-one care, the physical therapist stays with you for the entire session. This allows them to answer any questions you may have, monitor you throughout the session, and ensure that you're doing your exercises correctly. In addition, the plan of care may be adjusted based on how your symptoms present that day and on how you've progressed in your rehab. On the other hand, one-on-one care tends to be more common in cash-based clinics due to the insurance reimbursement rates in the US today. This is difficult to find in clinics that take insurance due to slim margins imposed on these businesses.

When you go to a PT clinic and work with a therapist and an aide, these businesses usually take insurance as payment. As a result, these PT sessions may be more accessible (more clinics that take insurance versus those that don't) and the cost per session is generally lower (but also dependent on each person's individual plan). However, using your insurance also poses barriers in and of itself. Most plans require a doctor's referral to PT and written approval from your insurance company, which may take weeks to process. As a result, the time between the onset of your pain and your first PT session may be 4-5 weeks. In addition, these clinics are then required to see more patients, and you'll likely also work with an aide to complete your exercises. Usually, patients directly interact with PTs on an average of 10-20 minutes out of the 60 minutes they're at the clinic.

At Valor Physical Therapy, we've taken all of this into account. Because our primary goal is to provide quality patient care to get patients moving pain-free faster and more efficiently, we've chosen the cash-based model. The one-on-one sessions allow for deeper assessments and real-time adjustments to the plan of care. We also want to ensure that PT is accessible for everyone who needs it, so we do offer a sliding scale on request and have partnered with Freehab Therapy, a non-profit that provides pro-bono therapy to those with neurological conditions. We're also constantly hosting free educational seminars and events at our clinic so that we can continue to help our community.

Expertise and Advanced Treatment Techniques

A clinic and the physical therapists there are defined by the depth of their clinical expertise. For athletes and active individuals, having a therapist who understands high-level biomechanics and strength and conditioning is essential. Looking for a provider who addresses the root cause of dysfunction rather than simply managing symptoms can be most helpful for patients. Much of physical therapy school focuses on the foundational knowledge in order to treat the general population. For those searching for more specialized care, look for PTs or clinics that offer expertise in the area you're concerned about.

Look for a clinic that publishes the specific techniques and skills their therapists are trained in, not just broad claims about quality care. For example, at Valor Physical Therapy, our approach to orthopedic physical therapy and sports physical therapy goes well beyond general rehabilitation. We utilize a comprehensive range of hands-on and evidence-based treatments, including:

  • Manual Therapy and Joint Mobilization
  • Myofascial Release and Soft Tissue Mobilization
  • Trigger Point Therapy
  • IASTM / Graston Technique
  • Cupping
  • Athletic Taping
  • Neuromuscular Re-Education
  • Plyometrics and Return-to-Sport Progressions
  • Compression Therapy and Cold Therapy
  • Balance and Proprioception Training

This breadth of modalities and treatment techniques ensures that your joints, tissues, and movement patterns are resilient enough to meet the demands of your lifestyle and sport. Having these "tools in our toolbox" helps us to provide more specific care for patients that may benefit from one or more of these aspects of treatment.

We also address different issues including:

Comprehensive Testing and Return-to-Sport Readiness

If you want to return to a physical activity that you love, search for a physical therapist or clinic that will also prioritize your goals and incorporate them into your treatment plan. For the general population, physical therapists focus on getting people moving without pain, especially with household or daily tasks. For the active individual, the goals need to be shifted even further. The period of time between pain-free with daily tasks and pain-free with sports is actually longer than you think. However, that period of time heavily dictates how successful you'll be in that sport and what your risk of re-injury really is. Progressively loading your joints, muscles, and tissues to withstand the demands of your sport is essential to setting you up for success. This involves careful programming and comprehensive testing to determine if you're truly ready to return to sport.

Because we see many athletes and active adults, Valor Physical Therapy uses advanced performance testing protocols to measure your progress accurately and guide clinical decision-making.

These include:

  • Movement Screens to identify hidden compensations
  • Force Plate Testing for objective strength, power, and stability data
  • Isokinetic / Dynamometer Testing for muscle symmetry analysis
  • Return-to-Sport Testing to confirm readiness for high-impact activity

Not only do we perform tests to ensure you're progressing in your rehab but we also have the tools in-house to get you there. Our facility utilizes a full turf area, dumbbell weight set, kettlebells, barbells, smith machine, squat rack, sled, cable machine, and much more. By effectively loading the body, we can help athletes return to their sport at a point where they're even stronger and more powerful than when they first came to our clinic.

Ultimately, it's your decision.

Choosing the right physical therapy clinic is one of the most important decisions you can make for your athletic longevity and daily quality of life. Before making an appointment, determine what you want for your care and see if that clinic or clinician matches your priorities.
